What do we know and not know about the first episodes of schizophrenia?

27. 3. 2025

This was the topic of another clinical seminar held in the Multifunctional Hall of the Centre for Mental Rehabilitation, which focused on the current knowledge about the first episodes of this disease and their treatment.

The seminar was opened by the psychiatrist of the Mental Rehabilitation Centre , Boris Dvořáček, MD, who offered an overview of the data on the treatment of schizophrenia at the Mental Rehabilitation Centre of the Beroun Hospital.

The guest speaker was psychiatrist and head of the research center for first episodes of SMI of the National Institute of Mental Health Filip Španiel, M.D., Ph.D. In his input he focused on the role of inflammation in this disease. "We have been collecting data on first episode patients for 15 years and have been following them for a long time. In that time, we have observed that inflammation in the body plays a big role in schizophrenia, and if left untreated, it can change the structure and functioning of the brain, leading to the negative symptoms we know in schizophrenia. This inflammation is very likely influenced by antipsychotic drugs, which is a surprise to us because we thought that these drugs worked differently. So now we can look at therapy in a different way and perhaps start to emphasise different approaches in clinical practice. By controlling the inflammatory foci, we can shift that trajectory away from what is common in schizophrenia and in the second phase we can look for new drugs that work a little differently than they have been working," says Spain.

Schizophrenia is a serious mental illness that affects a patient's thinking, emotions and behaviour. The first episodes of schizophrenia play a key role in the subsequent course of the disease, which is why early diagnosis and treatment are essential.

The seminar was organised by the Event and Community team of the Beroun Rehabilitation Hospital. Participants received a certificate of participation with a score according to the Regulations on Continuing Education of Clinical Psychologists.
